The mouthpiece has the words, Kellogg Switchboard & Supply CO. Chicago, U.S.A., and the bottom plate has numbers and letters, 951 C or GLR. The telephone is attached to an Oak Box that measures approximately 8 3/4" x 10 1/2" x 6 1/4" high. Inside the box 6 metal pieces. screwed into place, one top piece has the # 75.  The box has a black crank, it is attached to the wheel inside and turns freely. The top has one piece missing, and another piece that has come apart.  There are two, black, ringers on top of the oak box and a metal piece on top with the name of the company. The wires are still attached. The oak box is heavy, and final shipping costs to be determined on total weight and location.  I always liked this piece and hope some one else will as well.  I only ship to the United States.
